How it works

  2. Eligibility check. We verify Medicare or insurance before the first visit — no surprises.
  3. Start of care. A registered nurse visits within 48 hours and writes the plan of care.
  4. Visits. physical therapy visits follow the plan; progress is reported to the physician.

Questions about physical therapy

What does home physical therapy look like?

The therapist evaluates the patient in their own rooms, hallways and bathroom, then builds a program around real obstacles: the step at the front door, the tub edge, the distance to the mailbox. Visits are typically 45–60 minutes, two to three times a week.

How is this different from outpatient PT?

Outpatient clinics need the patient to travel. Home PT is for patients who cannot leave home without considerable effort, and it lets the therapist fix the actual hazards in the house.

Will therapy continue after home health ends?

When a patient no longer qualifies as homebound, we help transition them to outpatient therapy and send the full record to the physician.

Do you provide equipment?

Therapists assess for walkers, grab bars, shower chairs and other durable medical equipment and coordinate orders through the physician and a DME supplier.

Ready to start care, or have a patient to refer?

Call the office and a nurse will answer your questions. After hours, the same number reaches the on-call nurse. Physicians and discharge planners can fax orders to (310) 507-9989 or use the referral form.
